Skilled Nurse

 

At Advent Home Care Agency, Inc., we provide private duty/skilled nursing care for those with long-term needs or for anyone who is recovering after a recent injury, illness, or surgery. Patients receive care from registered nurses (RNs) or licensed practical nurses (LPNs) when services are ordered by a physician, and their needs go beyond the scope of care available from a certified home health aide. Private duty/skilled nursing is a convenient alternative to traveling to an outpatient center for treatment.

What Does a Private Duty/Skilled Nurse Do?

An at-home nurse manages, observes, treats and evaluates care, and takes the steps necessary to communicate with the prescribing MD in order to prevent an illness or condition from worsening. Advent Home Care nursing services include, but are not limited to:

Certified Personal Care Services

By providing a combination of oversight, supervision, companionship, and hands-on care, our Certified Personal Care Aides (PCAs) assist our clients to achieve their goal of independent community living. Our Registered Nurses provide individualized client assessments and supervision of our PCAs that ensures that our clients receive care that is safe and follows their individualized plan of care. Our team of caregivers, medical professionals, and administrative staff become each consumer’s teammates in their service provision.

Under New York State Department of Health regulations, Advent Home Care Agency Certified PCA’s may perform the following tasks for consumers: light housekeeping and cleaning specifically related to the needs of the consumer, meal planning and meal preparation, grocery shopping and errands with the consumer, laundry and assistance with personal care needs such as bathing, toileting, dressing, grooming, walking, medication prompts and reminders, assisting with transfers and routine skin care.
